If it is a story-line mission and not a random mission, then it might be the one where you have to eliminate the pirates in the system as well as escort the ship.
Or, you might have to be closer to the ship than you are.

It was a random mission and all it said was that the captain of the Jaguar received a death threat and needs escort.
May be there were *TWO* ships with the same name in the same system because the random name generator went crazy?! http://forum.shrapnelgames.com/images/smilies/rolleyes.gif

When you accept a random mission, won't it give the target ship a captain's name? The one there dosen't have one.

Were you supposed to be escorting a frigate, or some other class of ship?? And yep, it does give you that captains name, at least in any I've taken.

I took a similar mission today, and I can confirm that the name of the captain is NOT under the ship's name. The strange thing is, the moment I killed a pirate ship that got too close, a box poped up saying mission accomplished. I didn't fire a single shot in the Last one.

Note: The game does NOT check if it was you who killed the target. It only watches for the destruction of the target. If by some chance your target was killed by another AI ship you would get the credit for it. I have had that happen many times.

AFAIK, escort missions require you to destroy all pirate ships identified with a clan name.
