How can one get this message?
- It's a unique item and someone else has forged it?
- There are inherent chances of failure in forging all items?
- It's a random event?

Happens only when several mages try to forge the same artifact in the same turn. Of course only one of them will succeed since artifacts are unique.

* Bob was somehow feebleminded before the forge phase (assassin? seeking arrow?) and no longer had the magic skills to forge the item.
* Bob was relying on Forge of the Ancients to be able to forge that item, but FotA was dispelled.
* A random event caused you to lose some gems so you didn't have enough.
* A random event destroyed the lab in Bob's province; you can't forge without a lab.
