Does "Sacred" regeneration stack with spell regeneration?
Does it stack with item regeneration?
What about "inate" regeneration, like trolls?
Does item regeneration stack with other items?

Yes to all. Regen stacks.
And if you have a pretender with high nature magic, get even more regen for your buck.

Yes, as far as I know, but they need to be different items, i.e. no combining two rings of regen unless you have more than 2 misc slots and have something else in between.

That is something I never heard of before. I always thought you never got the benefit of a duplicate item.
Would you also get double quickness, for example, on a four arm Deva if there was an item in between duplicate items?

The limit is actually "if the item before this one is the same item, don't give bonus". This is because having more than two miscellanous slots is a relatively new feature, and few one-handed weapons have any effects worth taking a Nataraja for.
As long as the ability in question stacks, yes. However, Quickness (from the spell) is the spell's effect, and it's only ever applied once. Some of the XXX Brand-swords offer elemental resistances IIRC; they could be stacked if there was a shield between the Brands.

The bad thing about regeneration, even at 100%, is that you still get afflictions. It may reduce the likelihood, but it certainly doesn't seem, to me, to do so by a factor of 8. It would be nice if the percentage of regeneration your unit does, reduced your chances of getting afflictions too, or further.
