Does it work if I target an army that is moving ? Targetting the place where it comes from ? Or should I target the place where it's going to ?

There's a list of what happens in what order each turn.
The magic casting phase always happens before regular army movement, so you'll hit a normal army before it moves with something like rain of toads, fires from afar, or murderous winter.
I think that stealth armies might be a little different.

Well the spell description specifically mentions "stationed" so I wondered if the regular disease effect would have been changed by it.

It shouldn't. I'm 99% certain it just means "armies in the province" when it says stationed.

Target the province it's in. Magic happens before physical movement, so the only way for it to avoid the spell is to Astral Gate/Stygian Paths/etc. out (which will rarely happen) before you cast the spell. IIRC, casting order is random, so you've got a 50% chance of getting him before he teleports even if he does try this.
