Does penetration effect from two rune smashers stack? As I recall effects from two same misc items stack only if there's an unempty slot between them. Is it the same for the all types of items?

Your information on misc slots is out of date. Two of the same misc items don't stack anymore, whether there is something in between or not. That was fixed a couple of patches ago. There are some exceptions under special circumstances (2x Bracers of Defense + E9 blessing), but otherwise no. Two of the same weapon have also not granted any special abilities such as resistance, so twin rune smashers should not give doubled penetration bonus either.
