Is old age random? Why do some niefel gygas start with old age trait and some do not? Will some of them never get the old age trait?

the starting age for a unit has some variability. So a unit who's normal start age is close to their old age will sometimes start old and sometimes not.

all units will eventually get old - it just might take a few thousand turns ;)

edit: for instance, checking Edi's database, Gygia's have an old age at 300 and a normal start age of 330 - so most will start old.

a Skratti, otoh, has a normal start age of 243 and old age at 500, so you would never expect to see one old under normal situations. Some magic attacks will age a unit preternaturally fast.

Gygia's can have nature magic which would result in not having old age (by giving higher old age limit I think)

decay ages units but your gygja thing probably is nature magic related.. check it

Also some magic picks affect the aging of a caster. For a thorough explanation, take a look in the FAQ under "aging mechanics".

Generally, fire magic increases age or decreases max age,
nature magic increases max age.

Death path, makes getting afflicted from old age less likely.

Aging mechanics are covered in the FAQ and some units have built in variability, which can be great or small.

For example, EA Tien Chi Celestial Masters have about an even chance of starting old or not if they don't have nature magic. The EA Abysian misbreds have a variability range of around 200 years, so some could be young while most will be old.
