Weapon Accuracy

Something that has been a question for me for a while is how is the accuracy of the particular shot calculated? I have seen the penetration data base. Is there a formula that is used to determine the accuracy at a particular range? I am assuming that the weapons base accuracy is used but how does this accuracy degrade over range. The crew, fire control, stabilization and other factors are used also I am sure.
