Actually they can be reassigned.
And yes, as Taqwus said, insanity has no noticable effect on the non-commander units.

Something kind of interesting, if you've never been on the receiving end of crazy before - insane Rl'yeh commanders do random, usually non-helpful things, like calling their god back from the dead when the god is not in fact dead, preaching when they have no holy stat, researching with no magic stat, etc. But they do also declare themselves prophets, regardless of how many prophets you already have. This is highly useful. And sometimes the holy guys will preach and the mages will research - not necessarily what you want from them, but not so bad.
On the other hand, if you're not Rl'yeh and you go crazy, I've never seen a commander do anything useful during a crazy turn. They say interesting things like "that's no star" and "Hear my dreams of Rl'yeh", which I assume are just totally wasted turns.
So, even if crazy is bothering you as it spreads through your ranks of freespawn commanders, slave mages, and summoned Naiads, it's much, much worse for others.

You should keep an eye out for recently awake insane commanders. They will continue their insane actions if you don't reassign them (and unless they were insanely defending, which I haven't seen yet, they won't come up on the 'N' cue).

A funny moment related to insanity, when playing as LA Rlyeh took place when some enemy armies were besieging one of my land castle.
I broke the siege with my castle troops supported by nearby provinces incoming armies (like my powerfull pretender and some good summons coming from a void gates).
As it happened already several time in my insanes provinces, on the next turn in the troop assignement screen, several of the enemy soldiers (all insanes) were waiting to be assigned to one of my commanders, like if i bought them the turn before.
But what was a real surprise was the presence of a strange soldier (not commander) mounted on a horse that i never noticed existing as a soldier when being the other nation in a previous game.
And when right clicking to see the stats of it, i noticed it had the prophet icon : it was in fact the enemy pretender's prophet that decided to join me not as a commander but as a simple soldier , out of his own insanity
