There are some creatures that cause disease - most notably the Harvesters of Sorrow. You can also make the Bane Venom Charm that does the same thing. Both of these produced Messages, however. Several artifacts also cause disease on the wielder (as does the BVC) - Fever Fetish, Rod of the Leper King, etc.

There are random events and overland spells that cause disease, too.

2) Sounds like a bug. The poison icon also shows up for some other effects like Sulphur Haze, which requires Fire resistance to avoid damage.

1)No, he was always the only commander on my side when he fought. He was moslty casting priestly spells, so I don't think he ever got fatigue even over 100. He never wore any of the disease-causing artifacts. I guess it's possible that some enemy scout had BVC.
Are those overland spells that cause disease announced, i.e. do you get the message that the army has been struck by disease? I didn't get any of those, so if they are, that would be ruled out. And I think it's too early in the game for Harvester of sorrows.
So it's possible to get disease as a regular affliction in battle, even though the unit doing the damage has no special ability in its attacks? Not that he's been damaged, but I'm just curious.
Which of the diseases are infectuous, meaning that once one unit has them, they will spread to the rest of the army? How do I know how to distingush them?
2) He was fighting some militias, so I don't think there was anything special about it. As I said, the only thing I can see is that bog beast's poison spit counts as something else and not regular poison to which you can have a resistance.
