I think the nations are chosen by the user's preference, and in the event of a collision, a randomly selected person gets his first choice, the other picks a second choice.

As for "standard" settings, I've found that the usual MP-accepted settings are Indy-6, sites-50, HOF 15, standard richness and research, no graphs, renaming enabled (does anyone actually have a real reason to disable it? Why is this not just default by now?)
